Gets the practice a number at once with an earlier application's approved details (the number application read's reusable): a new application, approved straight away, then the number bought. The details cannot be changed; a change is a fresh application. agreedToCosts must be true, as for a fresh application. 409 billing_mandate_required without a Direct Debit; 409 unless the practice is awaiting a number and the details are still approved; 404 for an application the practice does not have.
