Import Time CSV

Re-uploads an analyzed time CSV with the selected row ids and the team member and
client name mappings, and creates a time entry per selected row. Entries go through
the standard create path, so charge and cost rates are stamped from the team member's
rate on the entry date and the practice's link requirement applies. Returns per-row
results; call it repeatedly with slices of the row ids to import a large file in batches.
