Creates the client in the practice's Sodium MTD tenant (provisioning the tenant on first use), makes the signed-in user a member, and links it to the client. Repeating it returns the same client. Needs the client to have reached the Party Service (409 until it has); refused (409) for a user restricted to some clients.
