Replaces the document's file with the uploaded one, in place on the practice's storage provider, and returns the updated document. The document keeps its code, title, category, folder, portal visibility and approval record, so links to it keep working, and the provider files the superseded content as an earlier revision reachable from the version list. Refused with a 400 for a document Sodium stores itself, or one on a platform that keeps no version history - there would be nowhere for the file being replaced to go.
